The focus is to understand both synchronous (occurring simultaneously with IPLC) and metachronous (occurring after an initial cancer) SPLC occurrences following surgery.<br /><br />The study included patients diagnosed with pathologic stage I IPLC detected via the NLST from 2002 to 2009, excluding those with non-malignant pathology. It evaluated the incidence rates of synchronous and metachronous SPLC, the 5-year cumulative incidence of metachronous SPLC, and survival outcomes, considering factors such as IPLC histology, smoking history, smoking status, and age.<br /><br />Key findings revealed that synchronous SPLC incidence was 3.0%, while the 5-year cumulative incidence for metachronous SPLC was 3.5%. It was observed that SPLC had differing stages at diagnosis and significantly varied long-term survival outcomes. Five-year overall survival for synchronous SPLC was 80.0%, whereas it was 53.9% for metachronous SPLC. Lung cancer-specific survival rates were 88.9% for synchronous and 61.5% for metachronous SPLC.<br /><br />The study highlights the importance of monitoring patients with resected screen-detected IPLC for secondary cancer occurrences. It provides valuable insights into the natural history and outcomes of SPLC after initial cancer resection, emphasizing the need for personalized follow-up strategies based on individual risk factors such as smoking history and cancer histology.
