P4.15E.01 Association Between Dynamic Patient-Reported Outcome-Based Symptom Exacerbation and Postoperative Complication in Lung Cancer

This study explores the association between dynamic changes in patient-reported symptoms and the risk of postoperative complications in lung cancer patients. Conducted at the Guangdong Provincial People’s Hospital, it involved 405 patients who underwent lung cancer surgery between January 2022 and February 2023. Using an electronic Patient-reported Outcome (ePRO) platform, patients' symptoms, such as sleep disturbances and shortness of breath, were tracked postoperatively. <br /><br />The study identified a significant correlation between symptom exacerbation on postoperative day two (POD2) and the occurrence of complications. Among the participants, 81 experienced postoperative complications, with 47 facing inpatient and 37 facing outpatient issues. A 10% increase in symptom scores was deemed the optimal threshold for predicting complications.<br /><br />Multivariate analysis revealed that the combination of sleep disturbances and shortness of breath on POD2 significantly increased the risk of complications, as shown by a Hazard Ratio (HR) of 3.231. Subgroup analysis highlighted a higher complication risk in females and patients undergoing robotic-assisted thoracoscopic surgery, with HRs of 3.217 and 5.532, respectively.<br /><br />The results suggest that exacerbations in certain symptoms can serve as early indicators of postoperative complications. Interventions aimed at managing sleep and respiratory symptoms could potentially reduce complication risks. The study also emphasizes the importance of enhanced follow-up care, especially for patients discharged from the hospital, to manage outpatient complications effectively. This research provides a basis for implementing early interventions and personalized follow-up strategies to improve recovery outcomes in lung cancer patients.
