P4.17F.05 Adherence to Clinical Practice Guideline - Analysis of 89 Multidisciplinary Team Discussed Patients with Stage III Lung Cancer
Back to course
Pdf Summary
The study evaluates the adherence of stage III lung cancer patients to clinical guidelines from the Chinese Society of Clinical Oncology (CSCO) post multidisciplinary team (MDT) discussions and examines the impact on treatment outcomes. The research involves 89 complex patient cases discussed by MDTs, primarily from Guangdong Lung Cancer Institute, encompassing over 80% of stage III and IV cancer patients. The aim is to determine how these discussions influence diagnosis and treatment plans, particularly striving for concordance with established guidelines.

Findings show that MDTs, incorporating various specialties in the diagnosis and treatment of lung cancer, are crucial for achieving adherence to the Standard of Care, especially given the complexity of stage III cases. These cross-hospital MDTs are effective in training lower-tier hospitals, leading to improved decision-making and increased guideline concordance. While there's no definitive best treatment for individual patients, the creation of a quality assurance tool might enhance guideline adherence in China, given the large patient population.

Results indicate a significant impact of MDT discussions on clinical decisions. For instance, 62% of cases saw changes in resectability evaluations, with some reclassified as potentially resectable or resectable. Furthermore, post-MDT, treatment plans were modified for 5-30% of cases to align better with guidelines or due to the lack of specific guideline recommendations for complex cases. There is consensus among healthcare professionals (HCPs) that MDTs aid in forming superior diagnosis and treatment strategies.

The study highlights the need for more cross-hospital MDTs and the implementation of digital quality assurance tools to assist lower-tier hospitals in aligning with standard care practices. This approach will ensure comprehensive and effective cancer care across diverse hospital tiers in China.
