Pdf Summary
The study explores the differences in tumor immune microenvironments (TIMEs) between primary and metastatic sites in patients with non-small cell lung cancer (NSCLC). The research, conducted by a team from institutions including Cedars-Sinai Medical Center and Tempus AI, Inc., utilized de-identified next-generation sequencing data from the Tempus real-world multimodal database. The study focused on data from 6,534 patients with metastatic NSCLC, analyzing RNA gene expression to estimate immune cell infiltration patterns in various tumor sites, including primary lung, liver, CNS, and bone.

Key findings highlighted that the TIME in liver, CNS, and bone sites is less immunogenic compared to primary lung tumors. This is significant as the TIME plays a crucial role in tumor progression and the response to immune checkpoint inhibitors (ICIs). Understanding the immune composition of these metastatic sites could influence the efficacy of ICI treatments. The study used multiple statistical tests, including Pearson’s Chi-squared and Kruskal-Wallis rank sum tests, to compare demographics, clinical characteristics, PD-L1 expression, tumor mutation burden (TMB), and proportions of immune cells like B cells, CD4 and CD8 T cells, NK cells, and macrophages across tumor sites.

Results showed that liver, CNS, and bone sites had a lower percentage of total immune cells and a lower proportion of B cells, CD4, and CD8 T cells, with a higher proportion of macrophages compared to primary lung tumors. The study advocates for further clinical trials to explore how differences in immune composition at metastatic sites may impact the effectiveness of ICIs in treating NSCLC. These insights could guide future clinical trial designs and help develop novel therapeutic strategies.
