EP.04C.04 Extended Inclusion Criteria for Lung Cancer Screening to Tabacco Related Diseases: Comparison of Preliminary Results in Two Prospective Cohorts

The authors hypothesize that incorporating smoking-related diseases could be a viable criterion for Lung Cancer Screening (LCS). <br /><br />The inclusion criteria for PREVALUNG ETOILE are people aged 45 to 75 with a history of smoking at least one cigarette per day for ten years, and a history of tobacco-related diseases such as atheromatous cardiovascular events, chronic bronchitis, or remissive tobacco-related cancers. Participants also met NLST or NELSON eligibility criteria. Exclusion criteria include symptoms of lung cancer, ongoing pulmonary nodule monitoring, fibrosis, pulmonary hypertension, severe cardiac or respiratory failure, and active pulmonary infectious disease. <br /><br />In the first round of LCS with a 5-month follow-up, the PREVALUNG ETOILE cohort included 160 participants. Among them, 127 met NELSON eligibility, 84 met NLST eligibility, and 133 underwent a CT scan. Of the scans, 22.5% were positive or intermediate, leading to five participants having minimally invasive diagnostic and curative surgeries. <br /><br />Comparative results showed the prevalence of primary lung cancer was 3.2% in the PREVALUNG cohort (14/434) and 1.5% in PREVALUNG ETOILE (2/133). For Stage I-II Non-Small Cell Lung Cancer (NSCLC), 71.4% were early-stage in PREVALUNG and 100% in PREVALUNG ETOILE.<br /><br />The preliminary conclusion suggests that extending LCS criteria to include individuals with tobacco-related diseases could effectively identify more candidates for early lung cancer detection. However, further research is necessary to establish statistical significance and validate these findings.
