EP.05A.04 Risk Factors for the Development of Pulmonary Infection after Bronchoscopy in Patients with Lung Cancer - A Centre in Greece

The study conducted at "ATTIKON" University General Hospital in Greece focused on identifying risk factors for pulmonary infections following diagnostic bronchoscopy in patients with lung cancer. Pulmonary infections after bronchoscopy are a serious concern as they can lead to treatment delays or cancellations and, in severe cases, death. In this retrospective study involving 182 adult patients diagnosed with lung cancer through bronchoscopy between January 2022 and April 2023, 7 patients (3.9%) developed a pulmonary infection post-procedure. The manifestations included pneumonia, parapneumonic effusion, and empyema. The onset of symptoms post-bronchoscopy ranged from 1 to 12 days, averaging 5 days.<br /><br />The study highlighted several significant risk factors for developing these infections. Five primary risk factors were identified: recent hospitalization for COVID-19, a personal history of Chronic Obstructive Pulmonary Disease (COPD), presence of emphysema on CT scans, endobronchial lesions causing substantial bronchial stenosis, and stage IV of Non-Small Cell Lung Cancer (NSCLC).<br /><br />Multivariate analysis showed strong associations, particularly indicating that recent COVID-19 hospitalization (OR: 64.88) and severe bronchial stenosis (OR: 9.6) had higher odds of leading to post-bronchoscopy pulmonary infections. The findings suggest that patients with these conditions face significantly higher risks.<br /><br />Ultimately, the study calls for standardizing the definitions of these complications through multicenter studies to improve the understanding and management of risk factors associated with pulmonary infections post-bronchoscopy. This understanding would help optimize the management and pre-procedure assessment to mitigate these risks effectively.
