Pdf Summary
The study conducted at the Nossa Senhora das Graças Hospital in Curitiba, Brazil, aimed to evaluate the effectiveness of endosonographic methods, specifically Endobronchial Ultrasound (EBUS) and Endoscopic Ultrasound (EUS), in staging thoracic neoplasms, with a particular focus on lung cancer. Lung cancer stands as one of the most common cancers globally and accurate staging is crucial for determining optimal treatment strategies.

The research involved a retrospective analysis of data from October 2021 to October 2023, examining medical records of 160 patients who underwent ultrasound evaluation of the mediastinum using EUS/EBUS methods. The study emphasized the importance of evaluating lymph node involvement, which significantly influences prognosis and treatment decisions. EBUS/EUS is noted as non-invasive and, according to previous meta-analyses, shows a 12% gain in sensitivity.

From the 160 patients evaluated, 110 had lymph node assessments through these methods, with 67% exhibiting no nodal involvement (N0) and 33% showing more advanced involvement (N2/N3). Of these, 38 patients proceeded to surgical procedures, allowing further validation of the EUS/EBUS results. The study found a specificity of 90.6% and a negative predictive value (NPV) of 93.5%, with an overall accuracy of 86.8%, aligning with existing literature on the effectiveness of these methods.

The findings underscore the competency of EBUS/EUS in staging thoracic neoplasms in this single-center study in Brazil, despite challenges in broader adoption due to health system limitations. The study’s methods and findings may encourage similar non-invasive diagnostic approaches in other centers, potentially enhancing diagnostic accuracy for lung cancer staging. The authors suggest expanding the research sample and include neoadjuvant therapy evaluations in future studies.
