EP.05A.07 Feasibility and Safety of Nasal Access for EBUS-TBNA in the Korean Population: A Single Center Experiences

This study explores the feasibility and safety of using nasal access for endobronchial ultrasound-guided transbronchial needle aspiration (EBUS-TBNA) among the Korean population. EBUS-TBNA is commonly used for diagnosing conditions such as mediastinal lymphadenopathy and lung lesions. Traditionally, the procedure is performed using an oral approach, but this research investigates the effectiveness of nasal insertion under conscious sedation. <br /><br />Conducted at Incheon St. Mary’s Hospital, the study retrospectively analyzed data from 1029 patients between March 2015 and March 2024. All subjects initially underwent EBUS via the nasal route, with the oral route used only if nasal insertion was unsuccessful. The focus was to evaluate whether the nasal approach could serve as a viable alternative to the oral method.<br /><br />The findings indicated a high success rate of 93.8% for the nasal approach. It was noted that younger females (under the age of 60) experienced a higher rate of failure with nasal insertion. Despite this, the nasal route required fewer analgesics compared to the oral route, although the level of conscious sedation (midazolam) was similar for both groups. Minimal complications occurred with nasal insertion, with epistaxis being the most common. Notably, only one severe instance of epistaxis required medical intervention.<br /><br />The study concludes that nasal insertion is a safe and feasible alternative to the oral route for EBUS-TBNA, potentially offering benefits for those with narrower nasal passages or higher pain sensitivity. As EBUS technology continues to improve, its application through nasal access is expected to broaden, enhancing patient comfort and procedural success.
