Pdf Summary
This study by Arisa Komatsu et al. investigates the impact of granulocyte colony-stimulating factor (G-CSF) administration on the risk of pneumonitis onset in patients with pre-existing interstitial lung disease (ILD) undergoing chemotherapy for thoracic malignancies. Conducted at Juntendo University Graduate School of Medicine, the research involved a retrospective analysis of patients treated with cytotoxic chemotherapy between September 2006 and January 2024.

The objective was to determine whether the use of G-CSF agents influenced the development of pneumonitis in these patients. While G-CSF agents are commonly used to manage chemotherapy-induced neutropenia, there have been concerns about their potential role in causing pneumonitis, particularly in patients with pre-existing ILD.

The study included 1,029 patients, and logistic regression models assessed various risk factors for pneumonitis. The analysis revealed that the presence of ILD, smoking habits, and male gender were significant risk factors for the onset of pneumonitis. However, the use of G-CSF agents was not found to significantly influence pneumonitis risk. Specifically, patients with pre-existing ILD did not show an increased risk when treated with G-CSF, with an odds ratio of 0.80 and a 95% confidence interval of 0.36-1.82.

Overall, the findings suggest that G-CSF administration does not significantly increase the risk of pneumonitis in patients with pre-existing ILD undergoing chemotherapy for thoracic malignancies. Other factors such as smoking and male gender were more prominent risk factors. This information could inform clinical decisions regarding the use of G-CSF in such patient populations, emphasizing consideration of other existing risk factors instead.
