Pdf Summary
The study by Y. Yazaki and colleagues from the Department of Thoracic Surgical Oncology, Cancer Institute Hospital of JFCR in Japan, explores the prognostic significance of serum carbohydrate antigen 125 (CA125) levels in patients with clinical node-negative (cN0) non-small cell lung cancer (NSCLC) undergoing surgical resection. CA125, commonly used as a tumor marker in clinical cancer treatment, was evaluated to assess its impact on patient outcomes.

A total of 2,883 patients were studied, divided into two groups based on their serum CA125 levels: one group with CA125 ≤ 15 U/mL (2,163 patients) and another with CA125 > 15 U/mL (720 patients). The study found notable differences in clinicopathologic features between the two groups. Those with higher CA125 levels had a higher proportion of males, smokers, and patients with larger tumor sizes. They also exhibited greater instances of lymphatic, vessel, and pleural invasion, along with higher rates of recurrence and worse survival outcomes.

The multivariate analysis indicated that elevated serum CA125 levels are an independent prognostic factor for both overall survival (OS) and recurrence-free survival (RFS). Specifically, patients with high CA125 demonstrated a lower 5-year OS rate (78.3% vs. 91.0%) and RFS rate (69.7% vs. 84.0%) compared to those with lower levels.

These findings suggest that serum CA125 levels can serve as an important prognostic marker for patients with cN0 NSCLC undergoing resection, assisting clinicians in stratifying risk and potentially guiding treatment strategies. The study advocates for the incorporation of CA125 level assessments in the prognostic evaluation of lung cancer patients post-resection.
