EP.06F.07 Comparison Between EGFR PCR Alone and NGS with Gene Fusion PCR for Detecting Oncogenic Driver Alteration in Thai NSCLC Patients

The study, conducted by researchers from Siriraj Hospital, Thailand, compared the effectiveness of EGFR PCR and next-generation sequencing (NGS) with gene fusion-panel PCR in detecting oncogenic driver alterations among Thai non-small cell lung cancer (NSCLC) patients. The study analyzed 155 NSCLC patients to evaluate the concordance of EGFR mutation detection between PCR and NGS and assessed the added benefit of using a gene fusion-panel PCR.<br /><br />Findings highlight that EGFR PCR has a faster turnaround time (average of 7.1 days) and lower cost compared to NGS (average of 19 days), making it efficient for initial screenings. However, EGFR PCR missed certain uncommon mutations which were identified by NGS. NGS detected mutations in 55.48% of cases compared to 53.55% by EGFR PCR. Despite EGFR PCR’s limitations in detecting rare mutations, its high concordance rate with NGS (Cohen’s kappa coefficient of 0.96) indicates it is reliable for most common EGFR mutations like exon 19 deletion and L858R mutation.<br /><br />NGS, while slower and more costly, was able to identify additional oncogenic alterations in 23.9% of patients. These included mutations in ALK, MET, ROS1, HER2, KRAS, BRAF, and RET, which might guide other molecularly targeted therapies.<br /><br />The study concludes that while EGFR PCR is suitable for identifying candidates for EGFR tyrosine kinase inhibitor treatments, NGS provides comprehensive mutation profiling, detecting uncommon alterations, and beneficially identifying additional actionable genetic changes in 11.4% of patients, thus recommending its use when EGFR PCR results are negative or when broader genomic profiling is needed.
