Pdf Summary
The study, presented by Satoshi Shiono and colleagues, investigates the differences in surgical outcomes between two segmentectomy approaches for early-stage non-small cell lung cancer (NSCLC): the hybrid approach, which involves a thoracotomy providing direct vision, and video-assisted thoracic surgery (VATS), a less invasive method with limited direct vision. The study includes data on patients who underwent segmentectomy between June 2016 and December 2021, with a total of 250 patients after exclusion criteria were applied.

Key findings highlighted differences in operative time, blood loss, and lymph node dissection between the two approaches. The hybrid approach yielded shorter operative times and less blood loss compared to VATS. It also resulted in a higher number of dissected lymph nodes, which might be indicative of better tumor staging and management. However, the study found no significant differences in perioperative outcomes, such as postoperative complications or the duration of hospital stay, between the two methods.

The study also employed propensity score matching to equate groups based on baseline characteristics, ensuring the comparison between the two groups was more scientifically valid. After matching, the overall survival rates were closely comparable, with the hybrid approach showing a 5-year overall survival (OS) of 92.2% and VATS at 90.8%.

Overall, the research concludes that while the hybrid approach is associated with certain intraoperative advantages, both approaches offer similar survival outcomes. The choice of surgical method could depend on specific clinical scenarios and surgeon preference, considering the advantages of efficient lymph node dissection and minimal intraoperative blood loss with the hybrid approach.
