Pdf Summary
The study evaluated the use of sublobar resection for early-stage non-small cell lung cancer (NSCLC) in a cohort of patients from the Mid-South region of the United States, following two pivotal randomized controlled trials—JCOG 0802 and CALGB 140503—that equated its outcomes to those of lobar resection. This retrospective analysis included patients from 2009 to 2023 across 14 hospitals in Arkansas, Mississippi, and Tennessee, focusing on those with stage IA NSCLC.

Key findings indicated that the overall use of sublobar resection, particularly segmentectomy, has increased significantly, especially following the publication of the JCOG 0802 trial in 2020. Out of 3,030 patients, 61% were deemed eligible for the trials, with 6.5% undergoing segmentectomy and 10.3% wedge resection. The increase in sublobar resections was predominantly observed among eligible patients, while changes among ineligible patients were not statistically significant.

The study underscores a positive adjustment in clinical practices towards utilizing sublobar resections in appropriately selected patients. However, there's room for improvement in reducing the application of such resections in ineligible patients. The consistent use among ineligible patients highlights ongoing challenges in aligning medical practice with evidence-based guidelines.

The research was conducted by Olawale Akinbobola and colleagues from institutions including the Baptist Cancer Center in Memphis, TN, and the University of Memphis. The study emphasizes refining patient selection to maximize the benefits of sublobar resection in suitable candidates, enhancing outcomes in the treatment of early-stage NSCLC.
