Pdf Summary
The study by Dr. Xing Wei investigates the differences in opioid use and pain control between thoracoscopic segmentectomy and lobectomy, two surgical procedures for non-small cell lung cancer (NSCLC). Utilizing data from the CN-PRO-Lung 3 longitudinal cohort study, the research focuses on patients who underwent video-assisted thoracoscopic surgery (VATS) segmentectomy or lobectomy from April 2021 to November 2022. Propensity score matching ensured comparable groups regarding demographics and clinical characteristics.

Key findings demonstrate that segmentectomy patients required a lower total dose of opioids both during and after surgery compared to those who had a lobectomy. Specifically, during surgery, the opioid dosage for segmentectomy was significantly lower (86.55 mg) than for lobectomy (100 mg). Postoperatively, segmentectomy patients used opioids less frequently (55.67% vs. 71.18%) and in lower amounts (24 mg vs. 36.92 mg), including cases utilizing patient-controlled analgesia (PCA), where segmentectomy still resulted in lower opioid doses.

Despite these differences in opioid use, both surgical groups exhibited similar postoperative symptom profiles. Assessment of pain, cough, fatigue, disturbed sleep, drowsiness, and distress from postoperative days 1 to 5 showed no significant differences between the two groups.

The conclusion suggests that segmentectomy is associated with a more conservative opioid usage profile without compromising the management of postoperative symptoms. Therefore, segmentectomy could be a more economical and effective option for NSCLC patients suitable for this procedure compared to lobectomy, highlighting its potential for reduced opioid exposure while maintaining effective symptom control.
