EP.07A.19 Impact of Preoperative Pulmonary Function on Survival Following Lobectomy for Stage I and II Lung Cancer

This study investigates how preoperative pulmonary function affects survival outcomes following lobectomy in patients with stage I and II lung cancer. The research, conducted at the University Medical Center Essen, spans from January 2016 to December 2022, analyzing all consecutive adult patients who underwent lobectomy for these cancer stages. The patient cohort was categorized into three groups based on their preoperative pulmonary function: Group A with normal function (FEV1 and DLCO ≥ 80% predicted), Group B with moderately reduced function (one or both values between 60-80% predicted), and Group C with severely reduced function (one or both values ≤ 60% predicted).<br /><br />The results revealed a significant correlation between preoperative lung function and five-year survival rates. Specifically, Group A had a survival rate of 77.2%, Group B had 59.9%, and Group C had 55.3%. Despite technological and surgical advancements, preoperative pulmonary function remains a critical determinant in the survival of lung cancer patients. Those with impaired lung function tended to have similar survival rates compared to those with moderately reduced function. <br /><br />The study emphasizes that patients with compromised pulmonary function should undergo meticulous evaluation and discussion in multidisciplinary tumor boards, rather than being automatically excluded from surgical consideration. This consideration is important because surgery could potentially extend patient survival. The research underlines the necessity for comprehensive preoperative assessments to guide surgical decisions, thus optimizing outcomes for lung cancer patients with varying degrees of pulmonary function impairment.
