Pdf Summary
The study explores the use of a portable Cone Beam Computed Tomography (CBCT) system for intraoperative marking of small pulmonary nodules, presenting a flexible solution to address limitations associated with traditional methods. Conducted by researchers at Kurume University, Japan, and presented at the 2024 World Conference on Lung Cancer, this study signifies the first official report on the application of portable CBCT in lung surgeries.

Traditional intraoperative marking using built-in CBCT systems is effective but has notable drawbacks, including the necessity for a specialized operating room and constraints on patient positioning. However, the portable CBCT system, specifically the Cios Spin (SIEMENS) paired with the MOT-VS600 operating table, alleviates these issues by offering mobility and adaptability in the operating room.

The study details its clinical application on four patients, demonstrating successful lesion localization in each case without complications. Advantages of the portable system include its ability for "anytime, anywhere" use and the flexibility to bend the surgical bed for additional resection, achieving a 100% lesion identification rate with satisfactory image quality. Nonetheless, the longer scanning time of 30 seconds compared to the conventional 10-second scan raises radiation dose concerns, prompting the need for further optimization.

The research highlights a surgical clip method for nodule marking as the simplest and safest approach, ensuring minimal patient stress. Future research directions involve optimizing the C-arm position reproduction, reducing radiation exposure, and addressing cost concerns. The study calls for continued corporate efforts to refine this innovative approach, marking a significant development in the field of thoracic surgery.
