Pdf Summary
The document focuses on the impact of radiation therapy (RT) on lung cancer patients with interstitial lung disease (ILD), particularly those with idiopathic pulmonary fibrosis (IPF). ILD is known to increase the risk of pulmonary complications post-radiation treatment in lung cancer patients. This retrospective study, approved by an Institutional Review Board (IRB), was conducted on 49 patients diagnosed with primary lung cancer and pre-existing IPF at Memorial Sloan Kettering Cancer Center (MSKCC) between 2000 and 2022. The study population ranged in age from 40 to 92, with a median age of 73, comprising 28 males and 21 females.

The primary objective was to evaluate the impact of RT on overall survival and pulmonary toxicity among lung cancer patients with ILD. The study revealed that patients with ILD are particularly at high risk for increased use of steroids and oxygen post-radiation therapy. For instance, after RT, oxygen use elevated from 52.6% to 73.7% for Stage I-II patients, and from 46.7% to 86.7% for Stage III-IV patients. Similarly, steroid use increased from 26.3% to 78.9% in Stage I-II and from 20% to 80% in Stage III-IV.

The survival rates observed align with existing literature on lung cancer patients with ILD. The study distinguished between cancer stages, showing a median survival of 1.05 years for Stage I-II and 0.68 years for Stage III-IV. The 1-year survival rate was 61.1% for Stage I-II patients and 42.9% for Stage III-IV, while the 2-year survival rate was 19.2% and 18.7%, respectively.

This study underscores the critical nature of managing pulmonary toxicity proactively in ILD patients undergoing RT for lung cancer to improve their quality of life and treatment outcomes.
