EP.07E.01 Intrinsic Impacts of the Expression of PD-L1 On Postoperative Recurrence in EGFR-Mutated Lung Adenocarcinoma

This study investigates the impact of programmed death-ligand 1 (PD-L1) expression on postoperative recurrence in patients with epidermal growth factor receptor (EGFR)-mutated lung adenocarcinoma. Conducted as a retrospective, single-center observational study at Matsusaka Municipal Hospital in Japan, it analyzed data from 221 patients who underwent surgical resection for stage A-A lung adenocarcinomas between 2017 and 2019. The study aimed to determine whether the combination of high PD-L1 expression and EGFR mutations contributes to recurrence risk and could guide postoperative management strategies.<br /><br />The study assessed recurrence-free survival (RFS) and overall survival (OS) using Kaplan-Meier analysis. Results indicated that PD-L1 positive patients showed a worse 5-year RFS compared to PD-L1 negative patients, but this was not statistically significant. PD-L1 status did not significantly impact OS. Additionally, while EGFR mutations were not significantly associated with 5-year RFS or OS, the combination of EGFR mutations and PD-L1 positivity was found to significantly correlate with poorer recurrence rates.<br /><br />Univariate and multivariate analyses suggested that PD-L1 status and pathological stage were significant predictors of postoperative recurrence. Notably, patients with predominantly papillary, acinar, solid, or micropapillary subtypes, and higher pathological stages, were more prone to recurrence if they were PD-L1 positive.<br /><br />The study highlights the complex role of PD-L1 in EGFR-mutated lung adenocarcinomas. Although the mechanism remains unclear, there is speculation that PD-L1 upregulation through EGFR pathways may impair T cell function, potentially contributing to tumor recurrence. Therefore, PD-L1 positivity is suggested as an independent risk factor for recurrence, necessitating further research to unravel its implications for patient management and prognosis.
