Pdf Summary
The EARLY-EGFR Thailand study investigates the prevalence of epidermal growth factor receptor mutations (EGFRm) in patients with early-stage non-small cell lung cancer (NSCLC) in Thailand. Conducted across 14 countries, the study focused on 145 patients, revealing key demographic and clinical insights. Lung cancer is notably prevalent in Thailand, with EGFR mutations being a significant oncogenic driver in advanced NSCLC. With 63.4% of early-stage Thai patients exhibiting EGFR mutations, the mutation rates in early-stage cases reflect those found in metastatic NSCLC.

The cohort mainly comprised females (70%) and never-smokers (77%), predominantly at stage I (57.9% at stage IA, 20% at stage IB). A large portion (69%) were identified through screening programs, with multidisciplinary team consultations being common pre- and post-surgery. Women showed a notably higher mutation rate than men (70.6% vs. 46.5%).

Among the mutations, Exon-19 deletions appeared in 44.6% of cases, while Exon-21 L858R mutations were present in 47.8% of the cohort. Surgical resection was the primary treatment, performed in 87.6% of patients, and lobectomy was the most common surgical procedure, resulting in a 98.6% complete resection rate.

Less than half of patients with stage IIA-IIIB disease received adjuvant systemic therapy, highlighting a need for better access to post-surgical treatments. Findings suggest the importance of early detection and treatment in improving prognoses for patients with early-stage NSCLC bearing EGFR mutations. The analysis emphasized that female gender was associated with increased mutation rates (adjusted OR, 2.99; p0.036). The study underscores the prevalence of these mutations in early stages, aligning with trends observed in more advanced cases.
