EP.07G.02 Nomogram Model for Predicting Risk of Spread Through Air Space in Stage IA Sub-centimeter Non-Small Cell Lung Cancer

The study aimed to construct and validate a nomogram model for predicting the risk of spread through air spaces (STAS) in patients with stage IA sub-centimeter non-small cell lung cancer. This model, developed using clinical characteristics and radiological features, serves as a prognostic tool for identifying STAS status preoperatively. <br /><br />A total of 112 patients who underwent surgical treatment for pathologically diagnosed stage IA sub-centimeter non-small cell lung cancer at Nanjing Drum Tower Hospital were retrospectively analyzed. These patients were divided into a training cohort and a validation cohort in a 7:3 ratio and further categorized into STAS positive and STAS negative groups based on their pathology results.<br /><br />Independent risk factors for STAS were identified through univariate and multivariate logistic regression analyses. These factors were then utilized to construct the nomogram. The nomogram's effectiveness was assessed using sensitivity and specificity measures derived from the Youden index. Additionally, the model's performance was evaluated using the area under the curve (AUC), calibration curves, and decision curve analysis (DCA).<br /><br />Results indicated that the nomogram demonstrated a good predictive performance for determining the STAS status in sub-centimeter lung cancer. This finding offers significant implications for preoperative planning for patients. By providing a clear prediction of STAS, this tool aids clinicians in making informed decisions regarding surgical interventions. <br /><br />Overall, this study underscores the utility of nomograms in enhancing personalized treatment approaches by accurately predicting disease spread in early-stage, sub-centimeter non-small cell lung cancer cases.
