EP.07G.07 Risk and Outcomes of Secondary Malignancy Among Non-Small Cell Lung Cancer Survivors Following Definitive Treatment

Non-small cell lung cancer (NSCLC) outcomes have improved, resulting in more long-term survivors. However, these survivors remain at significant risk for both disease recurrence and non-lung secondary malignancies (NLSM). This study, conducted at Memorial Sloan Kettering Cancer Center, aimed to characterize NLSMs and subsequent outcomes in NSCLC survivors.<br /><br />The study assessed 300 NSCLC survivors, focusing on those staged I-III who had completed definitive treatment and remained disease-free for at least 12 months. Over a median follow-up of 6 years, 17% experienced cancer recurrence. Additionally, 23% developed secondary malignancies, with a median onset at 58 months post-treatment. Of these, 10% developed NLSMs, typically diagnosed over 3 years after initial treatment.<br /><br />A significant finding was that 17% of patients with NLSM had metastatic disease, with a median overall survival (OS) of 6 months for these cases. For the broader group of patients with NLSMs, the median OS was 26 months, and median progression-free survival was 19 months.<br /><br />The risk of developing an NLSM increased over time, reaching 10% at 7 years post-treatment. The study suggests that many NLSMs are not detected by routine chest CT scans, emphasizing the need for comprehensive long-term surveillance strategies beyond standard practices.<br /><br />These insights underline the importance of continued vigilance for secondary malignancies in NSCLC survivors, proposing ongoing and possibly more comprehensive follow-up care to improve detection and management of NLSMs. The results highlight the necessity for alternative surveillance approaches to better address the risks faced by this patient population.
