This study evaluates the impact of locoregional recurrence (LR) and distant metastasis (DM) on overall survival (OS) in patients with non-small cell lung cancer (NSCLC) after surgery. It is a secondary analysis of a randomized controlled trial focusing on postoperative radiation therapy (PORT). The study included 364 patients, dividing them into PORT and non-PORT groups. The analysis found that both DM and LR significantly decreased OS in these patients, with DM emerging as the more dominant factor. In the non-PORT group, the odds ratio for DM's impact on OS was 4.74, while for LR, it was 2.09. Similarly, in the PORT group, DM had an odds ratio of 5.43, and LR had an odds ratio of 3.44.<br /><br />A multivariate Cox analysis identified critical risk factors for DM, including the pT stage, positive lymph nodes, and histology. A nomogram was developed to estimate the risk of DM. The study found that PORT did not significantly enhance OS in either the low-risk or high-risk DM subgroups but showed a benefit in the medium-risk subgroup.<br /><br />The results suggest that DM is a significant factor affecting survival and highlight the need for more effective DM control strategies. Meanwhile, PORT appears to be beneficial for those at a medium risk of DM, aligning with previous findings that PORT improves locoregional-free survival but not necessarily OS.<br /><br />In conclusion, the study emphasizes the importance of tailored strategies to address DM while also recognizing the potential for PORT to improve outcomes in specific patient subgroups. This highlights the complexity in treatment approaches for improving survival rates in NSCLC patients post-surgery.
