With the increasing use of autonomous suture devices in thoracoscopic surgery, staple line granulomas are an emerging issue, often mistaken for recurring malignant tumors during follow-up imaging. Correct differentiation is vital for clinical decisions, as treatments for these conditions differ significantly.<br /><br />The research involved 57 patients who underwent reoperation or biopsy due to suspicious nodules post-surgery from April 1995 to May 2022. The study excluded those who had lobectomy or pneumonectomy for their initial lung cancer. Of these patients, 21 were diagnosed with staple line granuloma, and 36 with recurrent tumors, based on pathological confirmations.<br /><br />CT imaging variables such as lesion size, attenuation, presence of internal low density, staple line abutting length, growth patterns, and more, were analyzed by three experienced readers using various CT scanners. The analysis revealed that significant CT features associated with staple line granuloma included internal low density and preserved vascular or lobular margin. In contrast, recurrent tumors often showed air-bronchograms and radial growth patterns.<br /><br />While the study's retrospective nature and lack of consideration for non-tuberculous mycobacterial (NTM) infection are noted limitations, it represents the largest cohort study for staple line granulomas to date. CT imaging insights could significantly help in making timely and accurate diagnoses, thereby facilitating proper management and treatment approaches and potentially improving patient outcomes.
