Pdf Summary
The study investigates the long-term effects of 'uncertain resection' (Run) in patients with surgically resected pathological N2 non-small cell lung cancer (NSCLC), a term introduced by the IASLC to describe ambiguous surgical outcomes beyond traditional classifications of R0 (complete), R1, and R2 (incomplete). The retrospective analysis included 155 patients, treated from 2016 to 2021 across three high-volume institutions. The cohort had a mean age of 67.2 years, with 39.4% female participants, and primarily consisted of patients who underwent a lobectomy.

Key findings reveal that 46.5% of patients had Run classifications, and these patients demonstrated significantly worse overall survival (OS) and disease-free survival (DFS) compared to R0 patients. The recurrence rate was higher in the Run group, though the recurrence site (loco-regional or distant) did not differ significantly between the two groups. Multivariable analysis identified older age, lack of adjuvant therapy, and Run classification as independent factors leading to poorer OS.

Moreover, the study found that Run patients had a worse prognosis even when adjuvant therapy was administered. Despite the nonsignificant statistical trend suggesting better outcomes for patients with single-station N2 involvement compared to multiple stations, R0 patients consistently exhibited better survival rates. The authors suggest that the Run category warrants enhanced postoperative management due to its association with a higher recurrence risk.

In conclusion, this study underscores the necessity for refined surgical and postoperative approaches in patients classified with Run status to improve their long-term outcomes. The findings advocate for a nuanced understanding and handling of 'uncertain resection' cases within NSCLC treatment protocols.
