Pdf Summary
The presentation discusses the use of Intensity Modulated Radiotherapy (IMRT) with a Simultaneous Integrated Boost (SIB) and adaptive radiotherapy (ART) for treating stage III non-small cell lung cancer (NSCLC). This study focused on 18 patients treated between July and December 2023, presenting findings from the dosimetric analysis at Chittaranjan National Cancer Institute, Kolkata.

IMRT with SIB allows higher doses to tumors while minimizing exposure to normal tissues. Adaptive radiotherapy (ART) is used to adjust for anatomical changes and tumor shrinkage during treatment, enhancing effectiveness and reducing unnecessary radiation to surrounding healthy tissue.

The study planned 66 Gy at 2 Gy per fraction to the gross tumor volume (PGTV) and 59.4 Gy at 1.8 Gy per fraction to the planning target volume (PTV) over 33 fractions, alongside platinum-based doublet chemotherapy. Initial and adaptive CT scans capture volumetric changes, crucial for the ART strategy.

Key observations included notable target volume shrinkage, with the median PGTV shrinking from 213 cubic cm to 146 cubic cm after 22 fractions. The study documented reductions in volumetric exposure and dose, particularly to the lungs, esophagus, and heart. Specifically, improvements included a decrease in V20, V30, and mean lung doses.

The conclusion emphasizes the potential survival benefits and dosimetric advantages of adaptive IMRT in lung cancer treatment. By reducing dose exposure to normal tissues while effectively targeting the tumor, ART may enhance clinical outcomes. These findings align with previous literature indicating the efficacy of adaptive techniques in radiotherapy for lung cancer.
