EP.09A.02 Addition of Pulsed Electric Field Ablation to SBRT for Lung Tumors: Effect on Health-related Quality of Life

The pilot clinical trial investigated the combination of Pulsed Electric Field (PEF) ablation with Stereotactic Body Radiation Therapy (SBRT) for the treatment of central lung tumors, which are challenging due to their proximity to vital structures and the increased risk of recurrence and toxicity with SBRT alone. This study aimed to assess changes in health-related quality of life (HRQoL) as a secondary endpoint.<br /><br />Participants included adults with up to three lung tumors, each measuring up to 6 cm, with a preference for central lung tumors. The treatment involved PEF ablation followed by SBRT, and HRQoL was measured using various subscales of the Functional Assessment of Cancer Therapy (FACT) both before treatment and three months post-treatment.<br /><br />Six patients with eight tumors participated in the study. The main findings revealed that there were no significant declines in HRQoL across the domains after the combined treatment. However, exploratory analyses indicated that a significant decrease in diffusion capacity of the lungs for carbon monoxide (DLCO) after treatment correlated with poorer functional well-being scores. This implies that maintaining DLCO levels could be critical for enhancing patient quality of life post-treatment.<br /><br />The trial concluded that the combination of PEF ablation and SBRT is well-tolerated, with patients reporting no significant HRQoL deterioration. Key insights suggest the value in strategies that aim to preserve pulmonary function post-SBRT to maintain HRQoL. This investigation highlights the potential of PEF ablation as a promising approach for treating lung tumors close to critical structures while managing patients' overall well-being.
