Pdf Summary
The study conducted by researchers from Guangdong Provincial People’s Hospital aimed to evaluate the safety and benefits of performing lung resection and cardiac valve surgery simultaneously under one anesthesia. This procedure was compared to performing the surgeries sequentially, examining outcomes related to economics, patient recovery, and overall experience.

From January 2018 to January 2023, 48 patients with a median age of 59.8 years were involved in the study, where a pulmonary tumor was unexpectedly found on chest CT scans before cardiovascular surgery. The study group consisted of 31 patients who underwent simultaneous surgeries and 17 who opted for sequential surgeries due to perceived risks. All participants provided informed consent.

The simultaneous surgery process involved a thoracoscopic wedge resection of the lung, a frozen biopsy, extracorporeal circulation for valve surgery, restoration of heartbeat, confirmation of malignancy, possible expansion to lobectomy, and completion with drainage tube placement and chest closure.

Results revealed that simultaneous lung and heart surgery is safe and cost-effective, with no perioperative deaths in either group. In the simultaneous surgery group, the majority were at clinical stage 1A (93.5%), while the sequential group had all patients at stage 1A. The 3-year survival rate (OS) for the simultaneous group was 96.2%, and progression-free survival (PFS) was 93.8%, which was comparable to the sequential group's rates of 94.1%.

Average hospitalization costs were lower for the simultaneous group (238,112 RMB) versus the sequential group (307,076 RMB), indicating significant cost savings. Overall, simultaneous surgery reduces anesthesia episodes, accelerates recovery, shortens waiting times, and lowers patient stress, positioning it as a beneficial option for candidates with concurrent pulmonary and heart valve issues.
