EP.09B.05 The Role of Pulmonary Metastasectomy (PM) For Non-Primary Lung Cancer: Umbrella Review of Meta-Analyses

This umbrella review evaluates the role of pulmonary metastasectomy (PM) in treating non-primary lung cancers, focusing on various primary cancer types. Although lung metastases in cancer patients are typically addressed through systemic treatments, PM is considered when the primary cancer is well-controlled, potentially offering cure and survival benefits. Common cancers for which PM is pursued include colorectal cancer, renal cell carcinoma, sarcoma, head and neck cancer, germ cell tumors, and breast cancer. However, despite its prevalence, PM's evidence base is limited due to challenges such as lack of control groups, diverse pre-surgery treatments, and varying follow-up protocols.<br /><br />The study utilized an umbrella review of meta-analyses to systematically evaluate PM's outcomes across diverse primary cancers. It extracted data from comprehensive searches in databases like PubMed/MEDLINE, EMBASE, and Scopus, focusing on English-language meta-analyses of PM for non-primary lung cancers. The primary outcome measured was the 5-year overall survival (OS) rate, while recurrence rates formed the secondary outcome.<br /><br />The findings revealed a pooled 5-year OS of 41%, with the highest OS in germ cell tumors (81.3%) and the lowest in melanoma (25.3%). The 10-year OS ranged from 24.5% to 26.9%. The recurrence rates post-PM were generally high, between 63.2% to 74.5% for most cancers, though notably lower in germ cell tumors at 26.5%.<br /><br />This comprehensive review highlights that while PM does provide fair clinical outcomes regardless of primary cancer types, its effectiveness varies significantly and further improvement may be possible with advancements like liquid biopsy integration. Yet, limitations such as insufficient data on complementary therapies and surgical modalities, and significant heterogeneity across meta-analyses, hinder a conclusive assessment.
