EP.11A.09 Predictive Role of BMI and Inflammation Markers in Immunotherapy (ICI) For HIV Associated Lung Cancer

Historically, people with HIV (PWH) have been excluded from immunotherapy trials, limiting available data on predictive markers. Despite antiretroviral therapy, PWH may have different immune and inflammation responses. Systemic inflammation markers are thought to be predictive in patients treated with immune checkpoint inhibitors (ICI) for solid tumors.<br /><br />The study includes patients with controlled HIV, where baseline characteristics did not show significant statistical differences across groups. It investigates the association of various factors like BMI and immune markers with overall survival (OS) in these patients.<br /><br />Key findings reveal that lower BMI is associated with shorter OS, while a high Systemic Inflammation Index (SII) correlates with longer OS in the ICI-treated cohort. Higher Monocyte to Lymphocyte Ratio (MLR) and Platelet to Lymphocyte Ratio (PLR) are also linked to longer OS in the overall PWH group. Notably, the Programmed cell death protein 1 ligand (PD-L1) score and CD4 counts did not correlate with OS.<br /><br />The study concludes that given the lack of a link between PD-L1 expression and clinical outcomes, better predictive biomarkers like BMI and SII are crucial for assessing survival outcomes in HIV-associated lung cancer treated with ICI. Future research should focus on systemic inflammation markers in larger cohorts to further understand their role in survival outcomes in this patient demographic. <br /><br />Overall, this research emphasizes the importance of understanding specific predictive markers in HIV-associated lung cancer to improve treatment outcomes for a previously underrepresented patient group.
