EP.11C.02 Role of Immune-Related Adverse Events (irAEs) and Albumin in Non-Small-Cell Lung Cancer (NSCLC)

This study explores the role of immune-related adverse events (irAEs) and baseline albumin levels in patients with non-small cell lung cancer (NSCLC) treated with immune checkpoint inhibitors (ICIs). The objective was to identify patient characteristics associated with irAEs and assess their impact on progression-free survival (PFS).<br /><br />Conducted at a single center, the retrospective study included 480 NSCLC patients who had received at least three doses of immunotherapy between 2015 and 2023. Patient demographics, treatment types, and clinical data were collected, with a focus on age, sex, smoking status, BMI, and baseline albumin levels. History of irAEs was also documented.<br /><br />Results indicated that 18.5% of patients experienced treatment-limiting irAEs, with pneumonitis as the most common. These adverse events were more common in females, non-current smokers, patients with higher BMI, and those on immunotherapy alone compared to those on a combination of chemoimmunotherapy. Univariate analysis demonstrated that patients with a history of irAEs had significantly better PFS (HR 0.534), and higher baseline albumin levels were also associated with improved PFS (HR 0.72).<br /><br />Multivariate analysis confirmed these findings, indicating that both irAEs history and increased albumin levels independently correlate with enhanced PFS.<br /><br />The study concludes that certain characteristics, including sex, smoking status, BMI, and treatment regimen, are associated with irAEs. Moreover, both a history of irAEs and elevated baseline albumin levels serve as potential prognostic factors for improved PFS in NSCLC patients undergoing immunotherapy. These findings may guide clinicians in predicting treatment outcomes and managing patient care more effectively.
