Pdf Summary
This case report by S. Yang and colleagues from the First Affiliated Hospital, Zhejiang University School of Medicine, discusses a rare instance of metastatic nuclear protein in testis (NC) carcinoma with a NUTM1::NSD3 mutation originating from the bronchus. NC is a poorly differentiated form of thoracic cancer characterized by fusion genes involving nuclear testis antigens, and NUTM1::NSD3 is one of its frequent variants.

The report highlights the challenge of treating metastatic NC due to its poor prognosis, with an average survival rate of 12 months. However, there have been instances where patients showed responses to immunotherapy, although the effectiveness of a combined approach with chemotherapy remains under evaluation.

The case study involves a 34-year-old male patient with a history of smoking, diagnosed with stage IIIB lung cancer. Genomic analysis revealed a NUTM1::NSD3 mutation through advanced sequencing methods. The treatment administered included a combination of albumin-bound paclitaxel, carboplatin, and the immunotherapy drug Tislelizumab. The patient exhibited significant tumor reduction following three cycles of this neoadjuvant therapy.

Subsequent surgical resection indicated a partial response, and further chemotherapy and radiotherapy were performed postoperatively. Impressively, the patient survived 10 months post-diagnosis without showing signs of recurrence or metastasis during follow-up.

The findings from this case underscore the potential advantages of combining immunotherapy with chemotherapy as an initial treatment strategy for such specific NC mutations. This could pave the way for enhanced therapeutic protocols when dealing with NCs harboring certain genetic profiles, thereby offering promising insights into personalized cancer treatment approaches.
