This case report discusses a rare instance of breast metastases originating from small cell lung carcinoma (SCLC), highlighting the complexities involved in diagnosing and treating such conditions. SCLC is a type of lung cancer accounting for approximately 15% of lung tumors and is often characterized by its systemic nature with frequent distant metastases at the time of diagnosis.<br /><br />The case involves a 73-year-old female patient first admitted in July 2021 for changes detected in the right lung lobe, subsequently diagnosed as SCLC. A CT scan revealed a mass in the right lung lobe along with enlarged lymph nodes. A bronchoscopy followed by a biopsy confirmed SCLC, stage T3N2M0. The patient underwent chemotherapy according to the PE protocol, followed by radiotherapy.<br /><br />Later, in November 2022, a CT scan for prophylactic cranial radiotherapy showed a secondary lesion in the brain. By January 2023, palliative radiotherapy was administered for cranial metastases. During this period, new changes involving the entire left breast and a lesion in the left anterior axillary line were identified. Cytological analysis confirmed that these were metastases of the original SCLC to the breast, an uncommon occurrence.<br /><br />The report underscores the necessity of distinguishing between primary breast malignancies and metastases to inform therapeutic strategies. Effective identification impacts treatment planning, as metastatic breast cancer may require a different approach compared to primary breast cancer. Despite being an unusual case, acknowledging such instances could provide critical insights for oncologists and guide treatment protocols for future cases of breast metastases originating from lung carcinoma.
