Pdf Summary
The study by Yusuf Kahya and colleagues from Ankara University's Faculty of Medicine examines the impact of the 9th TNM Staging System on clinical T-staging for pleural mesothelioma (PM), particularly focusing on the role of pleural thickness. Traditionally, pleural thickness has been the key factor for determining the clinical T-stage, yet the relevance of pleural effusion without thickening is not well documented. This research assesses survival outcomes in PM patients lacking pleural thickening but with pleural effusion.

The study evaluated 34 patients from 2009 to 2021 who underwent multimodal treatment, including Pleurectomy/Decortication. It considered variables such as age, gender, tumor localization, pleural conditions (thickening vs. effusion), histological subtypes, pathological stage, and overall survival using statistical tools like Kaplan-Meier analysis.

Findings indicated no significant differences between patients with pleural thickening and those with pleural effusion alone in terms of survival rates (overall 2-year survival was 63.3%, with 66.7% for the pleural effusion group and 62% for the thickening group, p=0.28). The research highlights the inconsistency between clinical and pathological staging in PM and suggests that pleural effusion alone does not affect survival detrimentally compared to pleural thickening.

The study concludes that clinical T-staging under the 9th TNM system might need revision to include pleural effusion as a distinct T-stage descriptor due to these findings. Such revisions aim to resolve the inconsistency and provide clearer survival distinctions across stages. The authors propose further studies to examine the feasibility of incorporating pleural effusion into the clinical staging criteria.
