EP.14A.09 Influence of Post-Pleurectomy/Decortication Peritoneal Dissemination Recurrence of Pleural Mesothelioma on Long-Term Survival

This study investigated the risk factors and survival prognosis of peritoneal dissemination following pleurectomy/decortication (P/D) in patients with pleural mesothelioma (PM). Conducted as a single-institution, retrospective study over a decade (January 2011 to December 2021), it analyzed data from a multimodality treatment program involving 272 eligible patients with epithelioid-type PM and specific clinical stage and performance status criteria. <br /><br />The study focused on patients who experienced recurrence after P/D, subdivided into two groups: those with peritoneal dissemination (P group, N=20) and those without (Non-P group, N=140). Data analysis used statistical techniques such as the Kaplan-Meier method and multivariable logistic regression.<br /><br />The results indicated that peritoneal dissemination recurred in one out of every eight cases and was particularly associated with being female (p=0.019) and undergoing diaphragm reconstruction (p=0.048). Postoperative recurrence involving peritoneal dissemination was linked to a poorer prognosis, with the P group showing a median survival of 6.7 months compared to 19.4 months in the Non-P group. Women and those who had diaphragm reconstruction showed significantly higher risks for peritoneal dissemination post-recurrence, indicated by multivariable analysis odds ratios of 3.7 and 2.8, respectively.<br /><br />The study highlighted that peritoneal dissemination recurrence is relatively frequent and negatively impacts survival, with specific risk factors identified to aid in patient evaluation. Furthermore, the analysis showed that postoperative surveillance strategies should consider these risk factors for better patient management and prognosis post-P/D. The researchers reported no conflicts of interest.
