Pdf Summary
The study investigates the role of serum-based inflammatory biomarkers, particularly platelet count, in determining tumor proliferation and prognosis in patients with malignant pleural mesothelioma (MPM). Conducted among 83 MPM patients diagnosed between 1998 and 2010, the research examines variables such as white blood cell count, neutrophil count, lymphocyte count, platelet count, C-reactive protein, neutrophil-to-lymphocyte ratio (NLR), platelet-to-lymphocyte ratio (PLR), and systemic immune-inflammation index (SII).

Key findings indicate a significant association between high platelet count and increased tumor proliferation, as evidenced by the Ki-67 proliferation index. Patients with high platelet counts exhibit poorer 5-year overall survival (OS) rates compared to those with lower counts, suggesting platelet count as a potential prognostic marker. Specifically, the high platelet group had a 7.1% 5-year OS compared to 24.4% in the low group. PLR and SII also showed relevance as predictors.

The study highlights that patients with elevated platelet levels may exhibit less favorable outcomes from chemotherapy and suggest alternative therapeutic strategies. High pretreatment platelet levels correlate with adverse outcomes, potentially increasing chemotherapy-related mortality.

The retrospective design and relatively small sample size may limit the study's conclusions, alongside treatment variability over the period and absence of certain data, such as immune checkpoint inhibitor therapy and consistent performance status records.

Ultimately, while platelet counts appear to have prognostic value in MPM, including a potential link to immunotherapy effectiveness, further research is required to develop targeted treatment strategies for patients with high platelet counts.
