EP.14A.14 Clinical Insights and Survival Outcomes of Malignant Pleural Mesothelioma: An Experience from a Tertiary Care Centre in India

This study aims to provide insights into the clinical characteristics and survival outcomes of Malignant Pleural Mesothelioma (MPM) from a tertiary care center in India. MPM remains a rare cancer, comprising only 0.13% of cancer incidences in India, with limited local literature available on its clinicopathological profile and treatment outcomes. The retrospective cohort study analyzed 25 patients treated at the All India Institute of Medical Sciences, New Delhi, between January 2016 and July 2023.<br /><br />The patient cohort comprised predominantly males, with a male-to-female ratio of 1.3 and a median age of 48 years. Notably, 32% reported asbestos exposure, although the condition predominantly affected younger individuals, potentially indicating genetic factors. A majority of the cases (64%) were detected at stage IV, predominantly of the epithelioid histological subtype (84%).<br /><br />Treatment strategies included platinum-pemetrexed-based chemotherapy, with no first-line use of immune checkpoint inhibitors. The response rate to treatment was 44.4%, and median survival was recorded at 26 months, aligning with global data. Treatment modalities varied, with 56% receiving chemotherapy alone, 28% best supportive care, and 16% undergoing surgery with chemotherapy. Subsequent treatment lines included agents like nivolumab, ipilimumab, and gemcitabine.<br /><br />The findings indicate a need for improved understanding of the biology and efficacy of treatment strategies for MPM due to its aggressive nature and poor prognosis. The study's outcomes call for further research to enhance therapeutic approaches and possibly incorporate advanced treatments like immune checkpoint inhibitors in the first line. Overall, the study underlines the exigent need for more robust clinical management practices for MPM in the Indian context.
