The 9th TNM classification has redefined how tumor size influences staging, now classifying T1 into T1a and T1b based on a 5cm threshold. Lung and diaphragmatic nerve involvement previously considered T3 has been reassigned to T2.<br /><br />Key findings highlight that patients with thymic tumors over 5 cm experience higher recurrence rates. A retrospective analysis using 3.9 cm as a threshold revealed significant differences in recurrence-free survival (RFS) and overall survival (OS). The 5-year RFS was 95% for tumors under 5 cm, compared to 83.1% for larger tumors. The 3.9 cm cutoff demonstrated an RFS of 92.0% for smaller tumors, significantly differing from 79.4% for larger ones. For the OS, those with tumors under 3.9 cm had 100% survival at five years, versus 81.9% for larger ones.<br /><br />Thymomas generally exhibit better prognoses than thymic carcinomas, with significantly higher 5-year RFS and OS rates. Prognostic factors include tumor size, WHO histology classification, and clinical stage. The data suggest potential refinements in future TNM classifications, such as further subdividing stage I.<br /><br />Conclusively, the study supports the 9th TNM system’s refinements, demonstrating tumor size as a critical prognostic factor. It indicates the possibility for future precision in staging, specifically subdividing stage I into more detailed categories. This research emphasizes the importance of tumor size in predicting patient outcomes, advocating for its integration into clinical practices for better management and treatment strategies.
