This retrospective multicenter study, conducted by the Mayo Clinic, aimed to evaluate the perioperative treatment experience for patients with thymic squamous cell carcinoma (TSCC). TSCC, a rare form of thymic carcinoma, predominantly occurs in the prevascular mediastinum and is often diagnosed at an advanced stage with a 5-year overall survival rate of only 55%. The study reviews outcomes from 1997 to 2023 for patients who had a surgical resection, assessing the impact of neoadjuvant treatments (preoperative treatments designed to shrink tumors) and the completeness of surgical resections on survival rates.<br /><br />The findings indicated that patients who received neoadjuvant treatments tended to have more advanced disease stages, which complicates direct comparisons between cohorts. However, these treatments resulted in a high rate of complete resections (68.12%), though 22.73% of patients still experienced progressive disease. It was observed that a higher stage of disease was significantly associated with poorer overall survival across all patients, as well as in those undergoing surgery first, with a statistical significance of p = 0.02. Additionally, in the neoadjuvant cohort, positive surgical margins (indicating that cancerous tissue may remain post-surgery) also correlated with worse survival outcomes, again with a significance of p = 0.02.<br /><br />In conclusion, the study suggests that employing neoadjuvant treatments in cases of advanced TSCC can improve the operability of tumors, thereby potentially enhancing surgical outcomes. These insights could guide treatment strategies, especially for patients with borderline resectable TSCC, to optimize surgical success and survival.
