Pdf Summary
This retrospective single-center observational study investigates predictors of 30-day mortality in hospitalized lung cancer patients who received systemic therapy. Conducted at the University Hospital of Parma, Italy, the study analyzed patients admitted from January 2017 to January 2022. It aimed to identify factors that could predict mortality within 30 days of treatment initiation, focusing on clinicopathological features and lab values at baseline.

Out of 144 lung cancer patients enrolled, 130 started anticancer treatment. The study found that 30-day mortality was 10%, with certain factors linked to a higher risk. Older age, non-smoking status, and non-small cell lung cancer (NSCLC) histology were significant predictors. Specifically, patients over 65, never smokers, and those with NSCLC showed higher mortality rates. Baseline laboratory indicators like low hemoglobin, low albumin, low pseudocholinesterase (pCHE), and low Prognostic Nutritional Index (PNI) were also associated with increased mortality.

Among the immunological scores evaluated, only the PNI was significantly correlated with higher 30-day mortality, emphasizing the role of nutritional status. Chemotherapy was the most common treatment, administered to 58.5% of patients, followed by chemo-immunotherapy.

The study suggests the need for careful consideration when administrating systemic therapy to older, non-smoking, and NSCLC patients with compromised baseline nutritional and laboratory profiles. These findings highlight the importance of assessing nutritional and immune status alongside traditional clinical factors to better predict short-term outcomes in hospitalized lung cancer patients receiving systemic therapies.
