Pdf Summary
The study conducted by Dr. Chukwuka Eze and colleagues at the University Hospital LMU Munich aimed to evaluate the effectiveness and safety of Magnetic Resonance (MR)-guided single-fraction Stereotactic Ablative Radiotherapy (SABR) for treating peripheral lung tumors. This prospective study recruited 29 patients with 33 tumors deemed inoperable, focusing on those with primary lung cancer and lung metastases. Treatment was delivered using a single fraction of 28 or 30 Gy guided by real-time MR imaging, with endpoints including local tumor control, overall survival, pulmonary function, inflammation markers, and quality of life (QoL).<br /><br />Key findings over a median follow-up of 19.2 months showed 1- and 2-year local control (LC) rates of 96.6% and overall survival (OS) rates of 89.5% and 76.1%. The study observed four cases of local failure, primarily in patients with colorectal carcinoma and Ewing sarcoma. Importantly, there were no significant grade 3 toxicities reported. Pulmonary function tests demonstrated minor reductions, notably a median 4.0% decline in forced expiratory volume over one second. Inflammatory markers showed an 8.3% increase in the neutrophil-to-lymphocyte ratio, whereas C-reactive protein and albumin levels held steady, and patient-reported QoL remained unchanged.<br /><br />Overall, the study concluded that MR-guided single-fraction SABR is a safe and effective treatment for peripheral lung tumors, offering high control and survival rates while provoking only mild impacts on lung function and inflammation. The findings emphasize the potential for dose-escalation strategies, particularly for patients with colorectal cancer lung metastases, and contribute valuable insights into physiological changes and patient experiences following this treatment.
