Video Summary
The webinar, “Highlights of ASCO 2026,” reviewed major lung cancer advances across targeted therapy and immunotherapy.<br /><br />Dr. Angel Chin highlighted three targeted-therapy studies. In LIBRETTO-432, adjuvant selpercatinib for resected RET-positive stage II–IIIA NSCLC dramatically improved event-free survival versus placebo, with only 4 events on selpercatinib versus 19 on placebo. In the 7-year CROWN update, lorlatinib continued to show durable first-line benefit in ALK-positive metastatic NSCLC, with median PFS still not reached and strong intracranial control. The phase 2 LOREN study suggested neoadjuvant lorlatinib has promising pathologic response rates in stage III ALK-positive disease, but key questions remain about optimal duration, the role of chemotherapy, and the best endpoints.<br /><br />Dr. Samuel Rosner discussed uncommon EGFR and small cell lung cancer. CRYSTAL-2 showed encouraging outcomes for first-line amivantamab plus lazertinib in atypical EGFR mutations, including compound alterations. WUKONG-20H found that sunvozertinib improved PFS over chemotherapy in EGFR exon 20 insertion NSCLC, though access and CNS activity remain concerns. In the DELPHI-304 post-hoc analysis, tarlatamab demonstrated notable intracranial activity in relapsed small cell lung cancer, with low rates of neurologic immune-related toxicity, though real-world management issues such as dysgeusia were discussed.<br /><br />Dr. Rebecca Heist presented three studies in EGFR-negative/driver-negative NSCLC. OPTI-1 showed sacituzumab govitecan plus pembrolizumab improved PFS over pembrolizumab alone in first-line PD-L1-positive disease. TRITON suggested tremelimumab/durvalumab plus chemotherapy may benefit STK11/KEAP1/KRAS-mutant nonsquamous NSCLC. HARMONY-6 showed the VEGF/PD-1 bispecific ivonescimab plus chemotherapy improved survival in squamous NSCLC, with ongoing questions about safety in central tumors and older patients.<br /><br />The session ended with discussion on adjuvant chemotherapy, treatment duration, MRD testing, toxicity management, and how to select patients for these new regimens.
