The webinar “Highlights of ASCO 2026” reviewed major lung cancer advances across targeted therapy, uncommon EGFR mutations, small cell lung cancer, and driver-negative disease.<br /><br />Dr. Chin highlighted three targeted-therapy studies. The phase 3 LIBRETTO-432 trial showed that adjuvant selpercatinib in resected RET-positive stage IB–IIIA NSCLC produced a dramatic event-free survival benefit versus placebo, with only 4 events versus 19 and a hazard ratio of 0.172. The 7-year CROWN update reinforced the long durability of lorlatinib in first-line ALK-positive metastatic NSCLC, with median PFS still not reached and strong intracranial control. The LOREN phase 2 neoadjuvant lorlatinib study in stage III ALK-positive disease showed a pathologic complete response rate of 47% and major pathologic response of 81%.<br /><br />Dr. Rosner discussed atypical EGFR and small cell lung cancer. CRISALIS-2 showed promising outcomes with amivantamab plus lazertinib in uncommon EGFR mutations, with median PFS of 19.5 months and OS of 41 months. WU-KONG20H found sunvozertinib improved PFS over chemotherapy in EGFR exon 20 insertion NSCLC. A post hoc analysis of DeLLphi-304 showed tarlatamab had meaningful intracranial activity in extensive-stage small cell lung cancer, including an intracranial complete response rate near 15%.<br /><br />Dr. Heist reviewed driver-negative disease. OPTI-TROP LUNG-05 showed sacituzumab tirumotecan plus pembrolizumab improved PFS over pembrolizumab alone. TRITON suggested possible benefit of dual checkpoint blockade plus chemotherapy in STK11/KEAP1/KRAS-mutant NSCLC. HARMONY-6 showed ivonescimab plus chemotherapy improved overall survival versus tislelizumab plus chemotherapy in squamous NSCLC.<br /><br />The discussion emphasized chemotherapy’s role in adjuvant settings, duration of TKI therapy, MRD use, toxicity management, and how best to integrate these new regimens into practice.
