Pdf Summary
The IASLC webinar reviewed emerging evidence and treatment strategies across small-cell and non-small-cell lung cancer.

In SWOG S1827/MAVERICK, brain MRI surveillance alone produced better cognitive failure-free survival than MRI surveillance plus prophylactic cranial irradiation (PCI) in patients with small-cell lung cancer (SCLC) and no brain metastases. Preliminary overall survival results showed no significant difference, while PCI reduced brain metastases but caused more treatment-related toxicity. The findings support MRI surveillance as a preferred approach, pending final survival results.

Two phase 3 trials of B7-H3 antibody-drug conjugates (ADCs) in relapsed SCLC—TAISHAN-302 and ARTEMIS-008—reported improved overall and progression-free survival compared with topotecan. Both showed manageable safety profiles, though interstitial lung disease rates differed and the China-only study populations limit generalizability. These results suggest B7-H3 ADCs could become important treatment options.

For EGFR exon 20 insertion NSCLC, REZILIENT 3 found that zipalertinib plus chemotherapy improved progression-free survival over chemotherapy, with substantial hematologic toxicity. Final PAPILLON results supported amivantamab plus chemotherapy, although overall survival improvement was not statistically significant, in part because many patients crossed over to amivantamab. Early phase 1 data for the EGFR/HER3 ADC iza-bren showed activity after prior EGFR-targeted therapy, warranting further study.

In the HARMONi-2 trial, ivonescimab improved progression-free and overall survival versus pembrolizumab in first-line PD-L1-positive advanced NSCLC in China. Toxicity was higher than with pembrolizumab, but no new safety signals emerged; a global phase 3 trial is underway.

Finally, early results for combining the investigational agents pumitamig and the B7-H3 ADC elfe-D showed encouraging responses in SCLC, including after DLL3-targeted therapy. Further follow-up and clinical trials are needed to establish the combination’s efficacy and safety.
