The webinar reviewed major WCLC lung cancer abstracts across small cell and non-small cell disease.<br /><br />For small cell lung cancer, the Maverick study compared prophylactic cranial irradiation (PCI) plus MRI surveillance versus MRI surveillance alone in patients without baseline brain metastases. MRI surveillance alone improved cognitive failure-free survival and had less toxicity, though PCI reduced brain metastases. Overall survival was not yet mature, but the data support MRI surveillance as preferred management, especially in extensive-stage disease.<br /><br />Two phase III Chinese trials tested B7-H3 antibody-drug conjugates in relapsed small cell lung cancer versus topotecan. Both showed clear overall survival and progression-free survival benefits with manageable safety, mainly hematologic toxicity and some ILD/pneumonitis. These results suggest ADCs may become the new second-line standard.<br /><br />In EGFR exon 20 insertion non-small cell lung cancer, zipalertinib plus chemotherapy improved progression-free survival and response rate in the RESILIENT-3 trial. Papillon final analysis showed amivantamab plus chemotherapy produced long-lasting benefit and improved patient-reported outcomes, with crossover complicating overall survival interpretation. An early-phase EGFR/HER3 ADC, EZABR, also showed promising activity in previously treated EGFR-mutant disease.<br /><br />For HER2-mutant NSCLC, DESTINY-Lung04 found trastuzumab deruxtecan significantly improved PFS and response rate versus pembrolizumab/chemotherapy, but overall survival was not improved and ILD remained a concern.<br /><br />Finally, Harmony 2 showed a chemo-free PD-L1/VEGF bispecific improved survival versus pembrolizumab in PD-L1-positive NSCLC, though safety and generalizability remain concerns. A palliative ADC/bispecific combination also showed encouraging early activity.
