Pdf Summary
The IASLC webinar reviewed emerging lung cancer evidence across small-cell and non-small-cell lung cancer (NSCLC).

In the phase 3 SWOG S1827/MAVERICK trial for small-cell lung cancer (SCLC), MRI brain surveillance without prophylactic cranial irradiation (PCI) improved cognitive failure-free survival compared with MRI plus PCI (HR 0.60). Preliminary overall survival showed no significant difference, while brain metastases were more frequent without PCI. MRI alone caused fewer treatment-related adverse events, supporting surveillance as a preferred approach, pending final survival results.

Two phase 3 trials of B7-H3 antibody-drug conjugates (ADCs) in relapsed SCLC—TAISHAN-302 and ARTEMIS-008—reported improved overall and progression-free survival versus topotecan. Both agents showed manageable safety, though interstitial lung disease/pneumonitis occurred and patient populations were limited to China, complicating cross-trial comparisons and generalizability.

For EGFR exon 20 insertion NSCLC, REZILIENT 3 found that adding zipalertinib to chemotherapy improved progression-free survival, with substantial grade 3 toxicity, mainly hematologic. Final PAPILLON results showed longer median overall survival with first-line amivantamab plus chemotherapy than chemotherapy alone, but the difference was not statistically significant, partly in the context of crossover. Early phase 1 results for the EGFR/HER3 ADC iza-bren showed activity after prior EGFR-targeted treatment; further trials are underway.

In HARMONi-2, first-line ivonescimab improved overall survival versus pembrolizumab in PD-L1-positive advanced NSCLC (30.8 vs 22.6 months; HR 0.73), alongside previously reported improvements in progression-free survival and response rate. Grade 3 or higher adverse events were more common, but no new safety signals emerged. Global evaluation is ongoing.

Early results for combining the PD-L1/VEGF-A bispecific antibody pumitamig with the B7-H3 ADC elfetabart drozuntecan showed encouraging responses in SCLC, including after DLL3-targeted therapy, with a manageable preliminary safety profile. Larger, longer-follow-up studies are needed to confirm these findings.
