EP.01.18 Family History of Lung Cancer in Lung Cancer in Never-Smokers, a Matched Case-Control Study

This multi-center retrospective matched case-control study investigated risk factors for non-small cell lung cancer (NSCLC) in never-smokers, a population not well represented in current lung cancer screening programs focused on high-risk smokers. The study included 6,000 participants, split evenly between NSCLC cases and controls, matched by age and sex. Key findings were that residence in non-capital regions, a history of chronic lung disease, and a first-degree family history of lung cancer were significant risk factors for NSCLC in never-smokers.<br /><br />Clinical characteristics showed no differences in age or sex distribution between cases and controls. However, cases were more likely to live outside the capital region (56.7% vs. 30.2%), have chronic lung disease (13.8% vs. 5.1%), and report a first-degree family history of lung cancer (11.1% vs. 9.1%). Multivariate logistic regression confirmed these as independent risk factors with odds ratios of approximately 3.0 for non-capital residence and chronic lung disease, and 1.2 for family history, all statistically significant (p < 0.05). Hypertension showed a slight protective association.<br /><br />Subgroup analyses revealed that the association between first-degree family history of lung cancer and NSCLC was particularly significant among males and individuals under 60 years of age. The study's results highlight the importance of considering family history and regional/environmental factors in lung cancer risk assessments for never-smokers, suggesting that these factors could enhance targeted screening strategies.<br /><br />This investigator-initiated study, funded by AstraZeneca, emphasizes the need for broader risk factor identification beyond smoking history to improve early detection and outcomes for lung cancer in never-smokers.
