EP.05.03 Analysis of Left Upper Pulmonary Arterial Bronchial Concomitant Consistency Based on Three-Dimensionalreconstruction

This study analyzes the variability and concomitant consistency of left upper pulmonary artery and bronchial branches using preoperative three-dimensional (3D) lung reconstruction in 300 patients with early-stage non-small cell lung cancer in the upper left lobe. The research aimed to clarify the relationship between arterial and bronchial anatomical variations, which is crucial for surgical planning, particularly segmentectomy.<br /><br />Key findings include that the most common cross-lobar arterial variant involved the A8 branch extending into the lingular region, observed in 67 cases (22.3%). Within this, A8 entered segment S5 in 19% of cases, S4 in 3%, and S4/5 in 0.3%. Notably, 43.2% of these cross-lobar A8 variants co-occurred with a fused fissure, a statistically significant association (P < 0.001). Variations in arterial and bronchial structure were most frequently seen in segments S12c and S3a. Significant correlations were found between the distribution of A12 branches and concomitant S12c artery and bronchus variations (P = 0.045), and between the common trunk configurations of A3a, A3b, and A3c branches and concurrent A3a artery-bronchus variations (P < 0.001). Additionally, the lingular segment’s most frequent arterial and bronchial variations involved two types of mediastinal lingual arteries.<br /><br />The study concludes that preoperative 3D reconstruction significantly enhances the understanding of left upper lobe artery and bronchus distribution and their associated variations. This detailed anatomical insight can improve surgical planning accuracy and the execution of lung segmentectomies for early-stage lung cancer, potentially reducing intraoperative risks and improving outcomes.
