EP.06.36 Non-Small Cell Lung Cancer With Diffuse Coexpression of TTF-1 and P40:A Clinicopathologic and Molecular Study of 19 Cases

This study investigates a rare subtype of non-small cell lung cancer (NSCLC) characterized by diffuse co-expression of TTF-1 and p40, analyzing 19 cases to elucidate their clinicopathological, immunohistochemical, and molecular features. Predominantly affecting older male smokers, two-thirds of patients presented with advanced-stage disease (stage III-IV). All tumors were poorly differentiated, often exhibiting basaloid or solid morphologies with biphenotypic immunohistochemical profiles positive for both TTF-1 and p40 markers.<br /><br />Molecular analysis revealed frequent mutations in TP53 and actionable alterations in genes such as MCL1, KRAS, and EGFR. These multigenic, targetable mutations highlight the necessity of comprehensive molecular testing to guide personalized therapy. Clinically, these NSCLCs display rapid progression and poor prognosis compared to conventional subtypes.<br /><br />The study emphasizes the distinct biological and clinical behavior of NSCLCs with co-expression of TTF-1 and p40, suggesting they represent a unique subtype with specific diagnostic and therapeutic implications. Future research directions include refining the classification of these rare biphenotypic tumors and exploring tailored treatment strategies based on their molecular profiles.<br /><br />In summary, NSCLCs co-expressing TTF-1 and p40 constitute an uncommon, aggressive group characterized by unique histopathology and genetic alterations, underscoring the critical role of integrative clinicopathologic and molecular evaluation for accurate diagnosis and management.
