EP.07.37 An Initial Experience of Resectable Non Small Cell Lung Cancer From a Tertiary Care Center

This retrospective study from a tertiary care center in western Rajasthan, India, reports initial experience managing resectable non-small cell lung cancer (NSCLC) from January 2020 to December 2024. Lung cancer is a leading cause of cancer mortality globally; local risk factors include exposure to mines, marble, and stone dust, alongside smoking.<br /><br />The study included 12 patients (8 men, 4 women) with a median age of 52.5 years. Most patients presented after a median of seven months of symptoms. While smoking combined with occupational exposure was a key risk factor, all female patients were non-smokers. Tumors were predominantly located in the upper lobes (6 patients), followed by lower lobes (4) and fissure involvement (2). Histologically, cases comprised five squamous cell carcinomas, four adenocarcinomas, two carcinoids, and one alveolar rhabdomyosarcoma—all non-small cell types.<br /><br />Surgical intervention included thoracotomy with mediastinal lymph node dissection. Ten patients underwent upfront surgery (lobectomy in 10, bilobectomy in 1, apical segmentectomy in 1), while two received neoadjuvant chemotherapy followed by surgery. The median tumor size was 4 cm, with eight patients staged early and node-negative, and four with locally advanced disease. Postoperative morbidity was limited to pneumonia (2 cases) and seroma (3 cases), with one mortality on postoperative day five. Median hospital stay was 10 days.<br /><br />Adjuvant treatment was provided to some patients: three received radiation (55 Gy over 5 weeks), and one underwent targeted therapy. At a median follow-up of 18 months, two patients experienced loco-regional relapse.<br /><br />The study concluded that multi-modal treatment with surgical R0 resection achieved relatively high overall survival at two years, with low complication rates, underscoring the value of a dedicated thoracic oncology team. Future goals include integrating all treatment modalities to enhance survival and quality of life.
