This retrospective audit studied treatment patterns and outcomes for locally advanced non-small cell lung cancer (NSCLC) in India, using medical records from 2014-2018 at major cancer centers. Out of 3715 registered lung cancer patients, 396 stage III NSCLC cases were included. The cohort comprised mainly squamous cell carcinoma (221 cases) and adenocarcinoma (124 cases). Survival analysis focused on 310 patients who received treatment, with curative intent in 240.<br /><br />Key findings showed that upfront surgery conferred superior overall survival (OS) and progression-free survival (PFS) compared to neoadjuvant chemotherapy (NACT) followed by surgery. Among non-surgical treatments, NACT followed by concurrent chemo-radiotherapy (CCRT) demonstrated better survival than either CCRT or sequential chemo-radiotherapy (SCRT) alone. Surgery plus adjuvant therapy yielded improved long-term outcomes compared to definitive chemo-radiotherapy. No significant survival difference was observed between CCRT and SCRT, likely due to a small sample size in the CCRT group.<br /><br />Treatment adherence was suboptimal, with only 56.7% adhering to overall protocols and 50.4% adhering specifically to chemotherapy regimens. Performance status and receipt of intended treatment were significant prognostic factors for survival on multivariate analysis.<br /><br />The study concludes that curative-intent therapy should be offered to eligible patients with locally advanced NSCLC. Resectability is associated with longer survival, highlighting the importance of early and appropriate surgical evaluation. The authors emphasize the need for standardized treatment protocols and well-equipped cancer centers to optimize care and reduce deviations from guidelines in real-world practice.<br /><br />In summary, this real-world Indian cohort confirms multidisciplinary management with surgery and chemo-radiotherapy remains essential for locally advanced NSCLC, with adherence to protocols and patient fitness strongly influencing outcomes.
